OK, OK, I know this form is REALLY long, but I need all this information to make a good page OK? Just a few rules/points:
  • Don't send me any spam/pranks with this form please, I'm trying to provide a service here!
  • When you e-mail me pictures, remember to tell me who is in the picture! don't just say "ok, here's my picture", or I won't know who's page to put it on!
  • If you don't have scanned logos or pictures... e-mail me and we'll talk about drawing a logo or maybe snail-mailing some pictures.
  • If you want me to add anything else (tour dates, extra comments and pictures or whatever), just e-mail me or add it in the extra comments box at the bottom.
